Enrolling Your Student

Welcome to Hart Public Schools!

Enrollment Information (2020-2021) 

 

All enrollment forms are available on our website (School>Resources>Documents and Forms)
or at the Central Business Office at 301 W Johnson Street in Hart.

 

You can either complete the forms in the office or take them home and return them once they have been completed.  
The Central Business Office will verify that you are a resident of the Hart Public Schools District. 
 
Please bring the following information with you:
  • An original or a copy of a certified student birth certificate (Vitalchek.com/birth-certificates# for a new birth certificate)
  • Photo ID of Parent/Guardian
  • Up-to-date immunization records
  • One residency verification document – such as a utility bill, tax statement, etc. (Sorry, we are not able to accept drivers licenses for this requirement.)

After you visit the Central Business Office, you will then proceed to the Transportation Building and finish by visiting your student’s school. The office will direct you.

Hart High School: 300 W Johnson Street, Hart (Grades 9-12)

Hart Middle School: 308 W Johnson Street, Hart (Grades 5-8)

Spitler Elementary School: 302 W Johnson Street, Hart (Grades K-4)

Diman-Wolf Early Childhood Center: 306 W Johnson Street, Hart (Pre K, Head Start and Great Start Readiness Program).                                     
*Please note enrollment for Head Start and GSRP take place directly at the Diman-Wolf Early Childhood Center. Please call (231) 873-6330
